When considering non-surgical rejuvenation an aging face, it is necessary that Dr. Glavey reviews the geometric dimensions of the patient's face. There are 3 dimensions that are taken into consideration: the "Y" axis, which is the total vertical height of the face from scalp to chin, the "X" axis, which is the measurement of the horizontal width of the face, and the "Z" axis, which if the forward (anterior) projection of the face from the insertion of the ears forwared. While the vertical ("Y" axis) dimension of the face rarely changes with time, the horizontal ("X" axis), decreases in width, and the Z axis decreases in size. Much of the loss of volume in the face is due to loss or movement of the fat pockets of the cheeks and temples. Loss of jaw volume and cheek fullness contributes to loss of the "Y"axis.
The second consideration in rejuvenation is the assessment of the consequences of long-term muscle activity. In essence, muscle activity which tends to draw the observer's eyes in an upward position is desirable. Muscle activity which tends to draw the eyes downward is undesirable. An example of this is seen when one observes the progressive downward movement of the corners of the mouth over time.
